اللَّهُمَّ أَصْلِحْ لِي دِينِيَ الَّذِي هُوَ عِصْمَةُ أَمْرِي وَأَصْلِحْ لِي دُنْيَايَ الَّتِي فِيهَا مَعَاشِي وَأَصْلِحْ لِي آخِرَتِيَ الَّتِي فِيهَا مَعَادِي
Al-lahumma ʾaṣliḥ lī dīniya alladhī huwa ʿiṣmatu ʾamrī wa-ʾaṣliḥ lī dunyāya allatī fīhā maʿāshī wa-ʾaṣliḥ lī ʾākhiratiya allatī fīhā maʿādī

When and why it was said
In Sahih Muslim (2720), it is reported that the Prophet ﷺ would supplicate for rectification of religion, worldly life, and the Hereafter.
